   Uniform Policy/House Rules Acknowledgment

• I understand; I will leave my ego at the door and treat everyone with respect.
• I understand; I will help foster a safe, fun, and clean training environment.
  • • I will keep my uniform clean, minimize foul odors, trim my nails, wear foot coverings in the bathroom, and maintain general clean hygiene.
• I understand; I will be responsible for myself and my family members by NOT STEPPING ON THE MAT with shoes.
• I understand; I must wear a white "Gi" (uniform) with an undershirt during the Trial Period. 
  • • If you do not have a Gi, Gracie Jiu-Jitsu North Phoenix will provide a new Gi at no cost.
• I understand; if I do not have a Gi, I will provide a security deposit for the value of the Gi provided by Gracie Jiu-Jitsu North Phoenix. 
  • • This is fully refundable if the Trial is canceled before the end of the 10-day Trial Term as outlined in the Trial Membership Agreement.
